The Pediatric Heart Center is subspecialty service line that specializes in the care of children with acquired and congenital cardiac disease. Surgeries and procedures include palliation and repair of complex congenital heart defects. Medical interventions include management of acute/chronic heart failure, cardiogenic shock, renal and respiratory failure. In our cardiac intensive care unit critically ill children are cared for by a dedicated team with expertise in mechanical ventilation, inhaled nitric oxide, ventricular assist devices, temporary pacemaker therapy, continuous renal replacement therapy and hemodialysis.
The Single Ventricle Complex Care APP (SVCC APP) will collaborate with the interdisciplinary Pediatric Heart Center team to coordinate complex discharges and support the safe outpatient care for infants and children with the most complex and potentially life-limiting pediatric congenital cardiac conditions, including but not limited to single ventricle physiology. The SVCC APP will work in tandem with the Medical Director of the Single Ventricle Home Monitoring Program.
Inpatient roles will include (1) coordinating and running weekly inpatient meetings with the home monitoring team and the parents, (2) providing direct and in-depth teaching of the home monitoring equipment and documentation, (3) working with bedside nurses to ensure appropriate reinforcement of teaching and equipment use, (4) tracking the progress of the patients towards discharge to assure all pre-discharge criteria are met and communicating such with the care team, (5) frequent documentation of the parents' and patient's progress in the EMR.
Outpatient roles will include (1) managing patients in the outpatient clinic who are transitioning at Family House prior to returning home (2) reviewing the weekly data submitted by the families, (3) conducting weekly Telehealth visits and documenting the visits in the EMR (including transcribing weekly data), (4) communicating summary of the Telehealth visit and weekly data with the Home Monitoring team and primary cardiologist by email, (5) managing the patients within the scope of the SVCC APN's experience (bloody stools, emesis, feeding mode and formula changes, wound infections, setting up appointments specialists) and relaying questions as appropriate to the Medical Director.
Inpatient and outpatient responsibilities will occur initially at the Mission Bay campus and extend to the Oakland campus in the future. In addition to these clinical duties, the SVCC NP will participate in, design, and implement improvement initiatives with the aim of improving the quality of care and survival rates for children with complex pediatric congenital cardiac conditions, including but not limited to single ventricle physiology. The SVCC APN will help design and maintain a Single Ventricle Home Monitoring website (with UCSF web developers support). The SVCC NP will also develop and oversee processes for the inventory and maintenance of patient care equipment owned by the department and/or Single Ventricle Home Monitoring Program.
The SVCC Nurse Practitioner will act as a full member of the team taking responsibility for the safe delivery of care in collaboration with attending physicians, fellows and residents. This team member will meet productivity expectations as set forth by the Division Chief, performing any duties related to patient care activities that are appropriate and clearly delineated.
